DMV driver history records are separate from court and criminal history records. Changes and time limits in your driver history printout do not affect any records kept by courts or law enforcement.

Driver Record Type. Type 1 Status record $4.00. Date of birth, license status, home address and original Texas driver license application date. Type 2 3-year history $6.00. Type 1 information, plus; List of crashes (if a ticket was received), and all moving violations for three years from date of offense. Type 2A Certified 3-year. Driver Identifier software is a relatively lightweight application that has been designed to quickly locate the most current drivers that are specific to your computer system, via a user friendly interface and access to an extensive database. Status - Indicates whether the license or ID is valid, expired, suspended, revoked or canceled.

Permit - Indicates whether the license is a normal driver license, an instruction permit or an ID card.

Issue Date - The latest date a valid license or card was issued. This is not the original issue date if you have renewed, received a duplicate or made any other change in your license. To receive proof of the date you first received a Nevada license, you must complete and an Application for Individual Record Information (IR 002).

Expiration Date - The date the license will expire. Licenses expire on your birthday four years after issuance or, if applicable, on the date of immigration expiration/departure. See Driver License Renewal Options.

Endorsements - Endorsement F is a limited commercial licensing exemption which allows firefighters, farm employees or active duty military members to drive large vehicles related to their duties. Endorsement J enables a Class C license holder to tow a vehicle over 10,000 pounds GVWR. Commercial drivers may hold many different endorsements including P for passengers.

Conviction Listing - These are listings of moving violations in which the motorist was convicted of the offense by a court of law. DMV is not notified of offenses which are dismissed or reduced to non-moving violations.

Demerit Points - The number of active demerit points. Points are assigned upon conviction of an offense and expire 12 months after the conviction date. An accumulation of 12 or more points within 12 months results in a license suspension. The first number in the violation code (Viol Code) indicates the number of points for that offense. Cite Date and Conv Date - The dates the citation (Cite) was issued by law enforcement and the date the motorist was convicted (Conv) by the court of jurisdiction.

State and Court - The state where the court of jurisdiction in the case is located and the DMV code for Nevada courts. Out-of-state courts are listed as 999. See the Online Code Search to find a specific court.

Viol Code and Off Ty - Violation code and offense type. These DMV codes are not the same as the codes listed on Nevada traffic citations. See the Online Code Search. Offense Type is the code reported to the national Problem Driver Pointer System and available to all 50 states.

In the example above, the driver was convicted of Driving Under the Influence (Viol Code 081) in the Las Vegas Justice Court (128). Three-year driver histories do not contain a record of license withdrawals. Hence, there is no indication of whether the license was revoked in this example.

CMV Off and Haz Mat - Indicates whether the offense was committed in a commercial motor vehicle (CMV) and whether the vehicle was carrying hazardous materials (Haz Mat).

Nevada has strict privacy laws. We do not accept driver history requests by telephone or email.

Original Issue Date - Uber/Lyft drivers who need their original driver's license issue date must complete and mail an IR-002 records application. See the mail option below. This date is not listed on an internet or kiosk driver history.

Former residents who need their license number or other information must also complete the application.

Employers, insurance companies and other third parties must open a records account. See Public Records Access.

The fee is $8.00. Your 3-year or 10-year history is delivered online while you wait. You must print this report at the time you use the service.

In Person & Kiosks

You may receive a 3-year, School Bus or 10-year history from self-service kiosks at larger DMV offices and a number of partner locations. See Kiosk Services.

Any DMV office may issue a history. There is no form to complete but you will need your license.

The fee is $8.00 plus a $1 kiosk processing fee. If you need a certified copy, contact the Records Section as listed below.

Records may be obtained from the DMV Records Section. You must complete and mail an Application for Individual Record Information (IR 002). The fee is $8.00. Please enclose a check or money order payable to DMV. If you need a certified copy, please enclose an additional $5.00 Certification Fee.

Crashes and ConvictionsTop ↑

Information about traffic crashes and convictions become part of your driving record. Traffic violations that occur in other states are added to your Nevada driving record if they result in a conviction.

Most violations are reported to your insurance company and employers for 3 years. A driver license suspension or revocation will be reported indefinitely if the license is not reinstated.

Drivers Club Founders

Information on most serious violations is available to all 50 state DMV's for a period of ten years after the conviction or license reinstatement.

Driver Finder

Convictions remain on file with the court which handled the case and in state and local criminal history files. Time limits and changes in your driving record do not affect these records in any way. The Nevada DPS Records, Communications and Compliance Division maintains Nevada criminal history records.
